Calibration needs roughly a week of regular practice before it is reliable. Until then difficulty is based on your diagnostic, which is a coarser signal.
Practice that is too easy feels productive and teaches nothing. Practice that is too hard produces a page of wrong answers and the conclusion that you are bad at the subject, which is usually not what happened.
The useful zone is narrow and moves as you improve, which is exactly the kind of bookkeeping software is good at and people are not.
Being "good at physics" is not a thing the exam tests. You are good at kinematics and poor at rotational motion, and a system that averages those gives you medium-difficulty questions in both — too easy in one, too hard in the other.
Difficulty is held per topic. That is why the map takes a couple of weeks to become reliable: it needs enough attempts in each topic separately, not enough attempts overall.
It is not teaching. If you have never covered a topic, adaptive difficulty will find the bottom of the range and keep you there while you get things wrong. That is a signal to go and learn it, not to keep practising.
It is also not a substitute for full-length papers. Practice sets build accuracy in a topic; only a full paper tells you what happens to your accuracy in hour three.
