Print at 100%. Any OMR reader must be shown this template once before it can process sheets in bulk — no generated sheet is automatically readable by arbitrary software, and anyone claiming otherwise is describing their own reader.
Most centres buying pre-printed OMR sheets are paying for paper and a layout they could produce themselves. The sheets are not complicated; what matters is that they print at exactly the right size and that the roll number field is unambiguous.
In any batch of two hundred, a few will not match a student — a misfilled roll number, a smudge, a student using last term’s number. The temptation is to discard them and publish.
Do not. Every unmatched sheet is a student who will ask why they are not on the rank list, and resolving four sheets takes ten minutes against a conversation with four parents that takes considerably longer.

Not a dedicated OMR machine — an ordinary document scanner handles coaching-scale volumes. You do need OMR reading software, and it needs to be shown this template once. A sheet is not self-describing; every reader matches against a template it has been given.

Yes — it goes into the instructions block, so a candidate deciding whether to guess can see what a wrong answer costs. The scheme is still applied at evaluation; printing it is about the candidate, not the reader.
Registration fiducials. A reader uses them to establish the page origin, rotation and scale, which is what lets it cope with a sheet fed slightly crooked. They are solid rather than outlined so they survive photocopying.
